SERVICES

Companion Services

We assist individuals who are age 18 or older to participate more meaningfully in home and community life. It is provided to individuals living in private homes for the purposes of providing supervision or assistance that is designed to ensure the individual’s health, safety and welfare or to perform activities of daily living for the individual.

In-Home Community Supports

This is a direct service (face-to-face) provided in home and community settings to assist participants in acquiring, maintaining, and improving self-help, domestic, socialization, and adaptive skills necessary to reside successfully in home and community-based settings. Services consist of support in the general areas of self-care, communication, fine and gross motor skills, mobility, personal adjustment, relationship development, socialization, and use of community resources.

Supported Living

In Supported Living, each person receives support to live in their own home, which they lease, rent, or own, and to acquire, maintain, or improve their skills to live more independently.
People may receive support in: Self-care, Health, Wellness, Meal preparation, Personal resource management, Communication, Transportation, Relationship development, Personal adjustment.

Homemaker/Chore

Homemaker Services include cleaning and laundry, meal preparation, and other general household care. Chore services consist of services needed to maintain the home in a clean, sanitary, and safe condition. Chore services consist of heavy household activities such as washing floors, windows, and walls; tacking down loose rugs and tiles; moving heavy items of furniture in order to provide safe access and egress; ice, snow, and/or leaf removal; and yard maintenance.

Residential

A licensed Community Home where services are provided to individuals with an intellectual disability or autism. To assist adult individuals with developmental disabilities, Autism, or dual-diagnosis to develop a lifestyle that follows those patterns and conditions of everyday life which are as close to the norms and patterns of "non-disabled" society.

Respite

Respite provides a break for the family from having to care for their loved one. Respite offers the best one-on-one care available to individuals with developmental disabilities, all in the comfort of their own environment. In-Home based care planned or a temporary care emergency can be provided for 2 hours + or overnight.
